Abstract

Fetal monitoring was carried out in 63 high risk pregnancies at Dr. Sardjito Hospital. Cardiotocographic examination (CTG) was used to evaluate 20 patients, and biochemical fetoplacental test. Urinary estriol (E3) and serum hPL were used to evaluate 43 patients. There were patients with postterm, preeciampsia, eclampsia and prolonged labor. Diagnostic test to predict fetal distress was analyzed for both CTG and biochemical test. CTG had a sensitivity of 87.5%, specificity of 58.3%, and accuracy of 70%. Urinary estriol level had a sensitivity of 25%, specificity of 89.7% and accuracy of 83.7%. The sensitivity, specificity and accuracy of serum hPL level were 25%, 92.3%, and 86%, respectively. There was correlation between relative birth weight and serum hPL level (r-z 0,3363), relative birth weight and urinary estriol (r=0.3427).Biophysical assessment Is more favourable compared to hormonal measurement in the detection of fetal distress.Key words: biophysical - hormonal assessment - fetal distress

Abstract

T-lymphocytes are the most important component of the immune response to control recurrent infections. T-Iumphocytes of CD4+ and CD8 + recognize a variety of viral proteins and produce lymphokines with antiviral and immunomodulatory effects. Both CD4+ and CIDER- bearing T-cells can kill HSV infected host cells. The relation between specific T-cells responses and severity of HSV disease have not been consistently detected. Interaction between T-cells responses and HSV and host cells result in a dynamic state of latency. HSV has evolved special mechanisms for evasion of host immunity. Reactivation can result in recurrences with the implication of transmission and/or disease. Molecular definition of T-cell responses for HSV may lead to immunological intervention to prevent HSV disease. Impaired T-cell immunity should be considered as a risk factor for severe infections.Key words: herpes simplex - T-cell role - latency - immunocompromised - vaccine

Abstract

In the surrroundings of mountainous area, many people are suffering from goiter due to low iodine contents in their diet. This study was aimed at examining T3, T4 and TSH concentration of the Norplant and IUD acceptors for 6 months. The acceptors were from an endemic goiter area in Cangkringan, Sleman, Yogyakarta. The results of this study showed that T3, T4 and TSH were in the normal range but relatively low level. Six months after using Norplant comparing with that using IUD, the T3 concentration decreased, while T4 and TSH concentration increased, but they were not significant (p>0,05). Based on those data, there was an evidence that after 6 months of using Norplant, the thyroid gland was hyperactive to produce tyroxine. After a long time period it would be a hyperplasia and hypertrophy of thyroid. Similar, further studies for a longer time are needed.Key words: Norplant - T3 - T4 - TSH - endemic goiter

Abstract

Muchlas 1 has adapted marital intimacy scale (10 questions) from 36-items of PAIR (Personal Assessment of Intimacy in Relationships) to suit with Javanese culture. In his dissertation, the scale was used to describe that one of the ways to increase the condition of mental health of mothers attending a course on mental health is by improving their marital intimacy. The result is not like what was expected, although the scale had been approved in the reliability and validity test. In his study on this scale, Sudiyanto2 said that although it had significant validity, the scale still had low internal consistency; there were normative questions, asking about morality instead of intimacy; several statements were inappropriate and the ten questions might not represent marital intimacy dimensions. The demand to revise this scale became urgent, as the scale is the only one planned for Indonesians and its practicality is required to evaluate the results of consultations on marital problems as well as mental health conducted by existing institutions, especially the Advisory Board of Marital and Separation Problems (BP4). Later Muchlas and Sudiyanto3 revised this scale of marital intimacy, and it became the revised scale of Marital Intimacy (15 questions). This new scale had better validation and internal consistency. In this study the revision of the scale of marital intimacy was used to measure the marital intimacy of worried patients who visited BP4 for consultation concerning with their separaty. Through an experimental study using psychotherapy, 100 random samples were divided into two groups; 50 samples belonged to a group provided with psychotherapy and 50 other samples belonged to a control group without psychotherapy. Their marital intimacy was measured using the new instrument before and after they were provided with psychotherapy. The result showed significant difference between the two groups and the validity of the marital intimacy scale used in this study was adequately good.Keyword : marital intimacy -- validity of the MIS -- REMIS -- psychotherapy

Abstract

A prospective study on 229 acute stroke patients was conducted to develop a Gadjah Mada Stroke Algorithm (GMSA). This algorithm was proposed as a clinical strategy for distinguishing intracerebral haemorrhage from acute ischaemic or infarction stroke after onset of stroke in 3 hospitals in Yogyakarta from 16th December 1989 until 15th November 1991. The following investigations have been made: (a) interobserver reliability for questionnaire and clinical examination of stroke patients, (b) interobserver reliability in the interpretation of CT-Scans of stroke patients, (c) validity of every clinical symptom against CT-Scans to define intracerebral haemorrhagic stroke, and (d) validity of seven multiple parallel tests against CT-Scans to define intracerebral haemorrhagic stroke. The GMSA was developed by one of seven multiparallel tests which has the highest validity. A multivariate statistical analysis and validate study showed that decreasing consciousness, headache, and Babinskis reflex at the onset are significantly related to intracerebral haemorrhagic stroke. This study showed that the GMSA was reliable and valid for distinguishing intracerebral haemorrhagic stroke from acute ischaemic or infarction stroke.Key words: Gadjah Mada stroke algorithm - diagnostic test - intracerebral haemorrhagic stroke

Abstract

Simple glaucomas are charaterized by an elevated intra ocular pressure (10P) and, are an important cause of blindness in the world. The blindness is due to the optic nerve atrophy and is found more frequently in women. It has been reported that high activity of G6PD isozyme can reduce the ocular pressure in the calf eyes. The activity of G6PD isozyme in the blood of simple glaucoma patients has been examined, and compared to those of normal persons. This study consisted of 76 patients of simple glaucoma (41 men and 35 women) and 30 normal persons as a control group. The 10P of both patients and control group was measured by using a Schiotz tonometer. The average of the G6PD isozyme activity was 9.40 ulg Hb in the simple glaucoma and 10.94 u/g Hb in control group. The two G6PD isozyme activity showed a significant difference. It can be concluded that the activity of G6PD isozyme of simple glaucoma patients were lower than those of normal persons without glaucoma symptoms.Key Words : isozyme - G6PD - glaucoma

Abstract

By bioassay Aedes aegypti larvae collected from Yogyakarta were found to be susceptible to temephos and malathion and by using microplate enzymatic assay Ae. aegypti larvae collected from the same sites were potentially resistant to the organophosphate insecticides due to elevated esterase activity in hydrolyzing a-naphthyl acetate substrate used in the enzymatic reaction from the same sites might be potentially resistant to OP insecticides due to the same mechanism. The homogenates of the mosquito adult stages from the fields were microassayed using a-naphthyl acetate substrate compared to that colonized in the laboratory. From a series of studies the results were concluded that Ae. aegypti adults in Yogyakarta were susceptible and potentially resistant to insecticides due to elevated a-naphthyl acetate esterase activity.Key words: Aedes aegypti - organophosphate insecticides - insecticide resistance - non-spesific esterase -biochemical test.
